Every patient is unique, and we are committed to providing you with high quality, state-of-the-art periodontal and implant care based on your individual needs. Above all else, we want to make your visit to our office a pleasant experience.

What to Bring on Your First Visit

If you received or downloaded our welcome packet prior to your first visit to our office, please bring the completed forms with you to your appointment. Otherwise, please arrive a few minutes early to allow time to complete the medical/dental health questionnaire, the patient information form, and review and sign the financial information form.

If you have a complicated health history, please mail it to our office so that we might review it in advance. Be sure to list all medications with dosages and any natural supplements, attaching an extra sheet of paper if necessary.

Also, bring your insurance cards for us to copy. If your general dentist has given you a written referral or x-rays, please remember to bring them with you to your appointment.

What to Expect on Your First Visit

Your first visit will include the following:

  • Review of your complete dental and medical history form
  • Review of your dental x-rays and/or taking new x-rays as required
  • Oral and soft tissue exam
  • Cancer screening
  • Occlusion and bite check

After a thorough exam, your periodontist will discuss his findings and answer any questions you may have about treatment. Our goal is to help you come to an informed decision regarding care.

If there is an urgent problem and you are currently a patient of record, please call our office and we will accommodate you as soon as possible. If the office is closed, you will be given specific instructions on how to obtain emergency advice or care.

Our goal is to provide periodontal treatment in a warm and caring environment. We understand financial pressures and our treatment recommendations tend to be conservative whenever possible. We also strive to make treatment affordable by keeping our fees reasonable and providing a variety of financial options.

Dental Insurance:

We will assist you in filing with most major private insurance plans and are a participating provider of Delta Dental Premiere Insurance.

Our goal is to ensure you receive the maximum allowable benefits given by your insurance plan and we will help you in determining what portion of your treatment is covered by insurance.

Please bring your insurance card or papers on the first visit.

Payment Options:

We accept most major credit cards (MasterCard, Visa, & Discover) and also will assist you in applying for outside interest free financing through Care Credit.

We will be glad to discuss these with you upon your arrival.
